What is ACL tear treatment?

During arthroscopic ACL reconstruction procedure, the orthopedic surgeon makes a small incision over the knee to assess and repair the ACL tear using an arthroscope. Severe ACL tear typically does not heal on its own and the patient needs prompt treatment to get rid of constant pain and discomfort. The orthopedic surgeon replaces the torn anterior cruciate ligament (ACL) by taking grafts from other parts of the body, such as the hamstring tendon, patellar tendon, or quadriceps tendon to restore joint function and stability.

What happens during ACL reconstruction treatment?

Diagnosis before ACL repair

The orthopedist conducts a thorough diagnosis that includes a physical examination. During the physical examination, the doctor assesses the tenderness, swelling, and inflammation in the affected area. The doctor may also suggest passive motion tests to check the severity of the torn ligament. There are a few diagnostic tests an orthopedist would recommend to get the clarity of the ACL tear in order to suggest the best-suited treatment to repair it. Some of the diagnostic tests are-
  1. Lachman’s Sign Test– In this test, the doctor holds the affected leg to bend it gently about 30 degrees. By doing this, the doctor can examine the intensity of the pain in the affected knee joint.
  2. X-Rays- This is usually recommended to get a clear image of the affected knee joint to check the injury on the bones.
  3. MRI- This is done to get a clear image of torn ligaments and tissues.

Procedure of  ACL reconstruction surgery

Treatment for ACL injuries can be performed via physiotherapy in some cases, but in most cases, surgery is the only viable treatment option to regain limb movement. ACL surgery, or ACL reconstruction, involves the surgeon removing the damaged ligament and replacing it with a tendon graft to restore complete joint function. Surgery is followed by intense physiotherapy to reduce the chances of re-injury, especially in athletes. Doctors evaluate the joint’s strength, stability, and movement patterns before allowing the patients to return to their sport. A longer rehabilitation period is often linked with a lower chance of recurrence of the injury.

Benefits of arthroscopic ACL reconstruction surgery

Arthroscopic ACL reconstruction surgery is a minimally invasive technique to repair a torn ligament. It is performed by an orthopedist specializing in arthroscopy. The success rate of arthroscopic ACL reconstruction is higher as compared to conventional surgical techniques. Minimally invasive surgery is often preferred over open surgery because it uses smaller incisions and causes less harm to the surrounding tissues, making recovery much smoother and faster. This surgical procedure can be performed on an outpatient basis and has a faster recovery duration. In addition, the patient can resume work within a shorter span of time in contrast to traditional ACL tear surgery.

Average cost of ACL surgery in India

The cost of ACL surgery ranges from Rs. 1,40,000 to 2,00,000. While selecting a surgeon or hospital for ACL surgery, cost is a big factor for consideration, as many patients often forgo surgery as they consider it expensive. However, ACL surgery cost is arbitrary and depends on the following factors:

  1. Type of anesthesia administered
  2. The city where the surgery is performed
  3. General cost of hospitalization
  4. General fees of the surgeon
  5. Comorbidities such as diabetes, liver or lung disease, etc.
  6. Type of graft used
  7. Additional surgery needed
  8. Rehabilitation and physiotherapy required
  9. Insurance coverage

What happens when you ignore an ACL injury?

Ligaments do not receive adequate blood supply to heal on their own. Therefore, ACL injuries do not heal on their own. If left untreated, they will progressively get worse and lead to:

  1. Instability of the knee joint: An injured ACL joint cannot sustain any weight and progressively becomes more and more unstable till the knee becomes too unstable to even walk.
  2. Pain: ACL injuries are riddled with pain. Without treatment, the pain increases over time, and the knee swells up so much that the person starts having difficulty even moving it.
  3. Disability: Without due treatment, the knee joint permanently loses its ability to bend and straighten, leading to permanent disability.
  4. Arthritis: As the ligament cannot function properly in an ACL injury, its functional load is born by the surrounding tissues such as bones, tendons, and muscles. This additional strain can promote arthritis development in many people.

What does rehabilitation for ACL injury for athletes entail?

Typically, rehabilitation for ACL injuries lasts for around 4-5 weeks, but since athletes put more strain on their joints, they need longer rehab periods for better movement and to prevent the recurrence of the injury.

Rehabilitation for knee injuries mainly focuses on treating the injury and providing the patient with the desired range of motion for the joint. The treatment is performed with three goals in mind:

  1. Stabilizing the joint to promote healing
  2. Strengthening the joint to increase the range of motion of the joint
  3. Increasing joint flexibility to decrease the chances of re-injury following the return to sports

Most athletes can start training for their sports about 4-6 months after the ACL surgery, but doctors recommend full rehabilitation before they start participating in the sports again. The rehabilitation period can be divided into:

  1. Phase 1: 3-4 months after surgery- Regain strength and improve the flexibility of the joint
  2. Phase 2: 4-6 months after surgery- Low-level fitness and performance agility drills
  3. Phase 3: 5-8 months after surgery- Jumping exercises
  4. Phase 4: 6-9 months after surgery- Single-leg jumping exercises with sports-specific cutting
  5. Phase 5: 9-12 months post-surgery- Steadily increase sports drills till former skill is achieved while focusing on preventing re-injury

Frequently Asked Questions on ACL Surgery

With proper physiotherapy, athletes can return to pivoting sports after 4 to 8 weeks, but for sports that put extra stress on the knee joint, complete recovery takes about 8 months. They have to make sure they have their orthopedic surgeon’s approval before returning to sports.

The necessity of ACL tear surgery depends on the type of injury. Partial tears can be managed using stretching and strengthening exercises. Therefore, for athletic people, surgery is recommended so they can resume their sports. Long-term instability of the knee can even promote arthritis, so it is crucial to keep up-to-date with strengthening exercises and wear a brace whenever you are moving your joint.

Swelling after knee arthroscopy is very common, especially near incisions. It usually dissipates within a few weeks with over-the-counter medication and ice massage. Physiotherapy after the surgery promotes recovery and helps resolve the swelling much faster. most cases.

The most commonly used grafts for repairing an ACL tear are patellar tendon autograft, patella tendon allograft, hamstring autograft, or a quadriceps tendon autograft.

Swimming is usually recommended after ACL surgery as it strengthens the quadriceps (hamstring muscle), which helps support the tendon graft. However, swimming and running should be avoided for at least 4-5 months after the surgery. If you want to, you can swim with just your arms without paddling your feet 2-3 months after the surgery with your doctor’s approval.

Being new to Hyderabad, luckily came to know about Doctor Puneet (Vascular Surgeon - Diabetic Foot care) through Google (Good) Reviews.Hospital was good with multi specialist doctors. Clean environment with seating facilities, restroom, drinking water. Receptionist, nurses, pharmacy staffs were so kind & helpful. As far as i know, it is Walk - In to meet any Doctor (First cum first served). Hospital is equipped with latest technology medical machine.Met Dr. Puneet Joopali for Diabetic Vascular issue for my elderly MIL. We were very happy & satisfied after consulting Doc. He speaks well, highly knowledgeable, easy to converse, explains well. Met him again to clear a doubt even without an appointment. He was kind enough to explain. I got his number & consulted him for general medical advice. He instantly replied over message. Very difficult to find doctors of this type who are easy to approach & get best advice. For Diabetics, Dr. Puneeth directed us to Dr. SVP Reddy. He was also easy to talk with, consulted well & with his medication sugar level is under control.Only one disadvantage we felt was that the prescribed tablets were Hyderabad manufactured & they were available only in hospital pharmacy. We couldn't get it in outside big pharmacy in Hyderabad city. Thanks to both doctors and also Max health Staffs.
